Have you tried formatting the SD card and reinstalling Rasbian - http://s.modmypi.com/noobs
This sounds like either a bad SD, or bad SD image install.

Hi Henry, The following guide demonstrates where the microSD card is on the Raspberry P. You'll need to remove it, by pulling it out of the microSD socket. Could you please swap the two LED's over side to side to ensure it's the LED that's faulty and not the board?

When I first installed the charge light was on, but I don't believe it was charging. the FSSD worked initially but only because the batt was fresh, I think. Could you please confirm your order ID for me?
You're running this on a "standard" version of Raspian?
Do you know if the battery has ever been charging?
Maybe try re-updating the firmware, and re-installing from scratch?
